WORK SCHEDULE IS MONDAY – FRIDAY 8AM – 5PM DURING TRAINING, AND AFTER TRAINING 8AM – 4:30PM WITH A HALF HOUR LUNCH, CENTRAL TIME ZONE. THIS ROLE IS SUPPORTING ILLINOIS MEDICAID BEHAVIORAL HEALTH MEMBERS. APPLICANTS MUST HOLD AN ACTIVE, CURRENT, NON-RESTRICTED LICENSURE FOR THE STATE OF ILLINOIS AS LISTED BELOW. Position Purpose: Develops, assesses, and facilitates complex care management activities for primarily mental and behavioral health needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families related to mental health and substance use disorder.
