PRN Home Health Occupational Therapist OT

Home Health Occupational Therapists (OT) responsibilities include:
- Conduct assessments to evaluate patients’ physical conditions and functional abilities
- Develop individualized treatment plans based on assessment findings and patient goals
- Provide hands-on therapy including therapeutic exercises, functional activities, and self‑care skill training
- Educate patients and families on proper techniques, safety, and rehabilitation strategies
- Collaborate with interdisciplinary healthcare professionals to coordinate patient care
- Maintain accurate and detailed documentation of patient progress, treatments, and plan updates
- Adjust treatment plans based on patient response and progress toward goals
- Attend staff meetings and in‑service programs as needed
Requirements for the Home Healthcare Occupational Therapist
- Valid Occupational Therapy license to practice as an OT
- Knowledge of medical terminology, physiology, and anatomy
- Experience in home health or a clinical setting (preferred but not required)
- Strong communication skills for working effectively with patients, families, and interdisciplinary healthcare teams
- Ability to work independently and as part of a team
- Compassionate, patient‑centered mindset with a genuine desire to help others
