What We’re Looking For
- Licensed Physical Therapist
- Degree from an accredited Physical Therapy program (DPT, MSPT, or BSPT)
- CPR certification (preferred)
- Experience in home care, Part A or B, or outpatient.
- Reliable transportation
- A compassionate, patient-centered mindset and strong communication skills
- Familiarity with electronic health record systems (e.g., WebPT or Hello Note)
- New Graduates are welcome to apply
Key Responsibilities:
- Evaluate and treat patients with physical disabilities, injuries, or functional limitations
- Develop and implement individualized physical therapy care plans
- Provide therapeutic exercises, manual therapy, gait training, and neuromuscular re-education
- Deliver orthopedic, post-surgical, and neurological rehabilitation services
- Educate patients and caregivers on home exercise programs and safe mobility practices
- Maintain documentation in electronic medical records in accordance with Medicare and CMS regulations
- Assist with fall prevention, balance training, and pain management strategies
