Key Responsibilities – What Your Typical Day Looks Like As a PRN Registered Nurse specializing in home health, you will be the linchpin ensuring that each patient receives safe, coordinated, and evidence‑based care. Document findings meticulously in electronic health records (EHR) and OASIS (Outcome and Assessment Information Set) forms. Care planning and execution: Develop and implement individualized care plans aligned with physician orders, Medicare home health regulations, and best‑practice clinical guidelines. Interdisciplinary collaboration: Communicate and coordinate with physicians, therapists, social workers, and home health aides to ensure continuity of care and timely intervention. Documentation excellence: Maintain accurate, up‑to‑date clinical notes, progress reports, and discharge summaries that reflect patient status, interventions, and outcomes. Regulatory compliance: Adhere to Medicare, ICD‑10 coding, and PPS requirements; ensure all documentation meets audit standards.
