Health Coach - Bilingual Spanish

How You’ll Contribute As a Health Coach, you’ll support patients in our virtual obesity program through ongoing coaching, education, and care coordination. You’ll play a key role in driving engagement, behavior change, and overall patient experience - working closely with providers and the care team. What You’ll Do
- Conduct regular virtual check-ins to track progress and motivate patients
- Support lifestyle, nutrition, and behavior change goals
- Serve as a primary point of contact for patient questions and support
- Coordinate with providers, dietitians, and care team members
- Document patient interactions in the EHR
- Escalate clinical concerns to licensed providers as needed
What We’re Looking For
- 1+ year experience in health coaching, patient support, or similar role
- Experience with obesity, chronic care, or behavior change (preferred)
- Health coaching certification preferred
- Telehealth experience required
- Health-related degree is a plus
- Strong communication and motivational interviewing skills
- Comfortable working in a virtual, team-based environment
- Tech-savvy (EHRs, telehealth tools)
- Private, HIPAA-compliant workspace
Schedule & Compensation
- Hours: ~20 hours/week, with flexibility based on availability and program needs.
- Hourly rate: $20-$28/hour, commensurate with experience.
